How to request assistance

When requesting assistance from the office, please include the following information. The more information you are able to provide, the more quickly and accurately we will be able to assist you.

Detailed Explanation:

Please provide Representative Andrews with the most thorough description of your situation in the clearest possible manner. Please submit complicated cases in writing, as this will ensure Representative Andrews can have a full and complete understanding of your concerns.

Supporting Documents:

Please provide any documents that support your case or may provide Representative Andrews with a better understanding of your issue. Should your case be referenced by a case, file or social security number, please provide that information as well.

Privacy:

Due to privacy laws, multiple state agencies are unable to speak to our office without a signed release form from you. Please see the Forms and Helpful Downloads section of this website to download a "Permission to Share Information Form", complete and email to Brian.Eno@mahouse.gov.   If you are asking for assistance with a MassHealth, Commonwealth Care or for a housing problem, please follow the above link so that you may complete the appropriate "Confidentiality Release Form" which will allow us to access information about your case.

Please note that Representative Andrews is unable to get involved in pending legal matters, nor is she able to provide you with any legal advice, as she is not an attorney.  As a State Representative, Representative Andrews may refer you to your respective United States Congressman or United States Senator when assistance is requested on a Federal matter.  Please do not hesitate to contact Representative Andrews should you have any difficulty locating the name or contact information of your federal representative.
