Please submit the generalized confidentiality release form when requesting assistance with housing and child support enforcement cases to ensure that Represenative Andrews is able to obtain information and make inquiries on your behalf. (click here to download form)
Represenative Andrews is happy to contact MassHealth and the Commonwealth Health Insurance Connector Authority regarding your concerns. However, it is compulsory to fill out this permission to share information form in order for her to make any such inquiries due to federal confidentiality guidelines. Please send the completed form back to Represenative Andrew's office (Room B1, State House, Boston, MA 02133-1053) by attaching them to an email to Brian.Eno@mahouse.gov along with a detailed description of your concerns. Please note that in "Section 3," it is necessary to fill in "Represenative Denise Andrews" as the person with whom you wish to share MassHealth information. (click here to dowload form)
Registering to vote in Massachussetts is a simple process. Please refer to the Elections Division website for simple instructions on how to register. You can also call the Elections Department directly at (617) 727-2828 or (800) 462-VOTE to have an application mailed to you. Please vote!
